Transition Planning and Aftercare Support

We know leaving care is a major life event, which we help children prepare for with care, thought, and encouragement.

Our Transition Planning and Aftercare Support ensures that every child who leaves us makes the smoothest possible transition into adulthood.

Our approach includes:

  • Developing transition plans as early as possible
  • Collaborating with social workers, families, and housing teams
  • Identifying future goals, aspirations, and life skills needs

Each child receives:

  • 1:1 coaching in budgeting, shopping, cooking, and self-care
  • Support in understanding tenancy, benefits, and employment
  • Emotional regulation strategies for coping with change
  • Help with accessing supported living, education, or apprenticeships

Our structured independence workbooks and tasks build a child’s confidence and competence in numerous ways. As their exit draws closer, they’re given gradual exposure to new environments, supported visits, and familiarisation sessions. Aftercare is available after they walk out the door, too. We maintain positive, ongoing relationships with former residents. They know they can call us, visit us, or check in whenever they need to.

Our aim is to:

  • Empower children to live independently
  • Minimise the risk of placement breakdown or homelessness
  • Help them build sustainable, connected futures

Individualised Care Plans

Every child has an individualised care plan developed in partnership with their key worker, family, and professional network. Daily life at Above and Beyond balances therapeutic support with structure and routine.

This includes:

  • 1:1 therapeutic keywork sessions
  • Access to formal or informal education
  • Recreational, cultural, and skill-building activities
  • Emotional regulation and independence coaching

We know healing happens through relationships. That’s why consistency, trust, and empathy are embedded into our daily care.

We welcome referrals from:

  • Local authority placement teams
  • Social workers and commissioners
  • Education and health professionals

Please send an email to referrals@aboveandbeyondrc.co.uk or call Sarah Nelson on 07380 420 115. Please provide supporting documentation (e.g., Education, Health and Care Plans and Risk Assessments). Our placements team will respond within 24 hours to discuss suitability and next steps.

OUR REFERRAL PROCESS

We welcome referrals for children aged 11 to 17 who require therapeutic residential care. Whether for emergency, short-term, or long-term placements, our referral process is responsive, transparent, and person-centred.
